Flu Vaccination Voucher Order Form

If you are 65 or older, you are eligible for a free influenza vaccine through the National Immunisation Program (NIP) and therefore you do not need a voucher, instead please book directly with your chosen/preferred pharmacy or see your GP. Please note that vouchers cannot be redeemed for National Immunisation Porgram (NIP) doses.

When ordering a flu vaccination voucher:

Please note:

Only 1 voucher is permitted to per person and in most cases your employer has to pay for the voucher you order whether you redeem it or not so please do not order one if you do not intend to use it
Your voucher code can only be redeemed at the booked pharmacy banner for one (1) flu vaccination appointment
Off-site clinics are available in all capital cities and some regional areas on selected dates between March – June
Available clinic date/s and time/s vary per locations
Clinic date/s and time/s are managed by the pharmacy banner/s directly, Medimobile has no control over available clinics
Vaccine stock is managed by the pharmacy group and is not controlled by Medimobile
You must be 16 years or older
To redeem your voucher for a flu vaccination you must book in online with the chosen pharmacy group (details of this will be available in your voucher order email which you will receive at the end of this process)
When booking and redeeming your flu vaccination voucher with the chosen pharmacy group you will need to enter your personal details including your medicare care card number (this is a requirement to ensure that the pharmacy group can record your vaccination to the Austrialian Immunisation Register as required by law).

Please note that WA and QLD are offering free flu vaccinations, so vouchers are not required. All employees in these states can book directly with your local pharmacy for free. Some participating pharmacies include Chemist Warehouse, Priceline, and Terry White Chemmart.
